Carol, you sound like me in the first few years, then it became bilateral
last  year after 6-7 yrs.  Dystonia can certainly be painful as your muscles are
being pulled  into abnormal positions.  My little left toe seems to be being
pulled out of its socket.

Some people just have dystonia.  One young gal had it so bad it broke her
jaw.There was young guy 18 yrs old here in Murrieta who was in fetal position 
due
to dystonia who was 'cured" by DBS.  Bilateral DBS only helped my tremors.
