Ask a local speech therapist about the LSVT (Lee Silverman Voice Therapy)
approach.  It's excellent, rigorous, requires much repeated practice over
time, but excellent.  Originating in Denver, in the past 5 years it seems to
have been offered now in many parts of the US, and there should be a
therapist familiar with it near you.
