We use a completely separate brochure for this purpose.  In Australia it is
called "Drugs to be used with Caution in Parkinson's Disease".  I believe
there are similar brochures in the USA.  A lot of work was done on this
topic several years ago.  We encourage every PWP to carry one with them all
the time, not just when they are going into hospital.  You might search the
archives for this form.  John Cottingham should know if it is there...
