You can choose your own doctor (assuming the doctor is taking Medicare patients) by getting out of the Medicare HMO and enrolling in traditional Medicare. There are limits regarding how often a Medicare beneficiary can switch form traditional Medicare to a Medicar HMO and back and forth. If you have made only one switch (from traditional Medicare to the Medicare HMO), then it should be pretty easy to switch back to traditional Medicare. I think you would have to stay in traditional Medicare for a minimum time period before opting a Medicare HMO again. Contact your local Social Security office.
