Patient/caregiver survey - CAN'T GET TO IT!

I tried to locate the patient/caregiver survey using both the www addresses
listed below. I also tried to find the gene article.  For whatever reason, I
got an ERROR message on several different times & I was denied access. It
appears to me, by the messages I received, that something has happened with
the PD site.
