I'm a doctoral student at Northern Illinois University and need
participants for my dissertation study. Also, I have Parkinson's
Disease. The survey title is "Lay caregivers insights into the needs
of Parkinson's disease patients".  I would like to ask PD caregivers
to e-mail their name and address, including zipcode, to me here at the
University. We will, in turn, send you a copy of the survey and a
postage-paid return envelope. No name appears on the survey and your
confidentiality is assured. Survey results will be sent to you if you
put your name and address on the back of the return envelope.
