Thanks to Hans, for digging up the very interesting article. At first glance, having skimmed but not really studied it, two comments: They say that R... is not very rare, just not always reported or diagnosed. Second, they name a long list of drugs that may cause R... Aside from circumstantial evidence such as a theoretical chemical reaction, they could not actually test each drug named on people, so how did they find that out for each one? When the condition is mentioned in the Package Insert for a drug such as Comtan or Tasmar, it's implied that they aren't even sure about a cause/effect relation for that particular drug.
