Dear Alvis Clegg:  Very interesting questions that you raise?  How much is
environmental and has there been anyone studying this?  Does anyone on the
network know of any epidemiology of this sort?  Maybe someone can do a
search of medline and see what could be found?  If nothing, perhaps we could
figure out how to get a study going.
