Medication response is one indication of PD.  i.e., the benefit from
Sinemet would indicate PD.  She did not elaborate about initial symptoms
and patient improvement.  Was there this occurrence?
The 50/100 CR referenced is not existent:  CR comes 25/100 or 50/200.  Was
there experimentation in dosage to establish minimum that gave good result?
Why was Eldepryl added?

I seem to read betwen the lines that anxiety is dominating this man and
that he is over-medicated rather quickly with potential that he is afraid
to the extent that he is anactive, depressed, and anxious but not
communicative.

Get him to a specialist Parkinsonian neurologist and talk about anxiety of
all concerned.  Reduced medication may be worth trying, - even a drug
holiday under close observance.  Does he sleep at night?

I do not know what the other medications effects might be.  What do l-dopa
and seligiline (Eldepryl) do to Meniere's symptoms?

These are conjectures of an educated parky trying to help.  A support group
might help daughter, mother and father-patient.  My limited experience, but
widespread reading has not shown this rapid a decline except for MPTP or
other toxic exposure - not normal PD.
